Registered Nurse
Nurse Responsibilities:
- Participate in the development and implementation of service plans and initial assessments to ensure the appropriate placement of staff.
- Regularly evaluate the needs of clients receiving nursing services.
- Assess and document observations of the patient’s condition.
- Dress or redress wounds and assess the progress of recovery.
- Educate patients and their families on proper care procedures.
- Collaborate with Physicians, Physical Therapists, Occupational Therapists, and other healthcare professionals in the patient’s care plan.
- Provide hands-on care, including wound care and ventilator care.
- Suggest tools or devices that could enhance the quality of life for patients.
- Conduct regular evaluations and re-evaluations of the patient’s nursing needs, with a minimum frequency of every 60 days or as warranted by changes in condition.
- Receive and implement verbal orders.
- Make referrals to other disciplines based on patient assessments.
- Maintain an up-to-date and accurate patient medication profile.
- Provide teaching on medication administration.
- Monitor medication usage.
- Complete service plans.
- Supervise Certified Nursing Assistants (CNAs), Personal Care Assistants (PCAs), and fellow nurses.
- Conduct training sessions for new staff.
- Oversee the management of records.
- Investigate incidents and grievances.
- Investigate accidents.
- Conduct orientation training.
- Communicate changes in visit assignments, scheduled visit dates, and schedule modifications promptly and effectively with the agency to ensure patient needs are addressed.
- Meet recurring training requirements and provide staff training based on assessments and regulatory requirements.
- 2 years work experience

Licensed Practical Nurse
LPN Responsibilities:
- Deliver professional nursing services under the guidance of an RN, adhering to the state’s Nurse Practice Act, relevant licensure/certification requirements, healthcare standards, governmental laws and regulations, and ARAA Home Care, LLC policies and procedures.
- Provide individualized care in a home setting.
- Implement the physician-prescribed care plan and meticulously document care in the record-keeping system.
- Administer skilled nursing interventions using a comprehensive approach aligned with the care plan.
- Apply knowledge and skills according to accepted clinical practice standards to facilitate issue resolution and attain patient-specific goals and outcomes.
- Consult with the RN Supervisor to discuss the treatment plan, accept physician orders, and convey them to the RN Supervisor.
- Employ a systematic, outcome-driven approach to implement the care plan on an individualized basis.
- Maintain regular communication with patients, physicians, clinical manager(s), and other healthcare team members promptly regarding patient/family needs and status changes, participating in care coordination as needed.
- Uphold the highest standards of professional conduct regarding confidential information, sharing information only when the recipient’s access rights are established and sharing is clearly in the patient’s best interests.
- Effectively communicate to ensure adherence to professional standards in the provision and availability of necessary supplies, materials, and equipment for the safe and effective implementation of the care plan.
- Complete and maintain documentation as required by ARAA Home Care, LLC and/or the facility, with documented visit/shift notes.
- Participate in the quality improvement program of ARAA Home Care, LLC.

Certified Nursing Assistant
We are an expanding company with multiple open positions for a qualified Certified Nursing Assistant (CNA). Your responsibilities will include delivering exceptional patient care and providing physical assistance to patients and residents on a daily basis. To excel in this role, you should possess a state-certified nursing assistant certification and have completed a nursing degree from a recognized CNA program.
CNA Responsibilities:
- Provide excellent patient care.
- Respond to patient calls and determine the best course of action to assist them.
- Offer physical support for daily activities, including personal hygiene tasks such as bathing, dressing, toileting, walking, standing, or exercising.
- Turn and reposition bedridden patients.
- Ensure adherence to prescribed diets by reviewing patient dietary restrictions, food allergies, and preferences.
- Gather information from physicians, caregivers, and nurses.
- Measure and document urinary and fecal output, reporting any changes to the medical staff.
- Record vital signs, including blood pressure, pulse, temperature, and respiration rate.
- Identify issues requiring medical attention, such as open wounds, bruises, or blood in the urine.
- Remind patients to take prescribed medications and supplements.
- Observe and report patient behavior, including complaints or physical symptoms, to the nursing staff.
